We have provided these sample letters for use by employees who are applying for parental leave from their employers.

We have left some details for you to change to fit your circumstances; this is where the text is [bracketed and in italics].  Where this occurs please either change the detail to reflect your circumstance, or insert the appropriate paragraph of text from the options available.

Letters for birth mothers taking leave

• Birth mother with 12 months eligibility, who is not transferring any leave to her partner
• Birth mother with 12 months eligibility, who is transferring a portion of her leave to a partner with 12 months eligibility
• Birth mother with 12 months eligibility, who is transferring a portion of her leave to a partner with 6 months eligibility
• Birth mother with 6 months eligibility - not transferring leave
• Birth mother with 6 months eligibility, transferring leave to a partner with 12 months eligibility to leave
• Birth mother with 6 months eligibility, who is transferring leave to a partner with 6 months eligibility

Letters for partners/spouses taking leave

• Partners/spouse letter - 6 months eligible service
• Partners/spouse letter - 12 months eligible service

Letters for adoptive primary carers taking leave

• Adoptive primary carer - transferring leave
